Tallaght Hospital's 'policy' of altering records:

Tallaght Hospital CEO Mr D. Slevin approves the falsification of medical records:

On 21st July 2014 I lodged a formal complaint with Mr D. Slevin the CEO of Tallaght Hospital.

My complaint was that the hospital's Dr J. Noel had falsified my medical records and the proof I lodged was the two contradictory documents signed by Dr Noel.

The first was dated the day of my consultation and confirmed to my G.P. the truth which was, that he had offered me treatment ASAP depending on when a bed became available - which never came about.

The second was dated 7 weeks later, in which Dr Noel now falsely claimed that at the consultation he had offered me immediate treatment which I had refused.

The first placed to blame / onus on the hospital, the second passed the blame on to me - this is very important as I'm left crippled.

Of note is that the second document appeared out of thin air over a year after its claimed date of writing.

On 29th July 2014 Mr Slevin refused to accept my complaint.

On 11th Aug 2014 I wrote to Mr Slevin challenging his refusal but he ignored me. This proves complete indifference by Mr Slevin to record tampering by his hospital.

A very important point is that Tallaght Hospital has already committed offences by deliberately releasing, against my wishes, my 'medical records' to third parties.

This and the falsification were just part of a hospital cover-up approved at the highest level.

Despite this extremely serious misconduct and against my expressly written instructions, the hospital insisted that I attend during Aug 2014 and see Dr Noel. I made Mr Slevin aware of this without interest being taken.

I have given Mr Slevin the opportunity to respond.
